Ways to explain your concerns and plan a more predictable first appointment.
Start with what worries you
Dental anxiety can relate to needles, sounds, previous experiences, embarrassment or feeling unable to stop a procedure. Some people find the unfamiliar setting harder than the treatment itself. The Oral Health Foundation recommends telling the dental team about your fear so that they understand your needs.
Try to identify one or two particular concerns. “I become anxious when I cannot see what is happening” gives the dentist different information from “I am worried about being judged because I have not attended for years.” Both deserve a clear response.
You can also say that you are unsure what triggers the feeling. The conversation does not have to begin with a diagnosis or a detailed account of your past.
Make the first appointment predictable
When booking, ask whether the first visit can focus on discussion, examination and planning. Confirm what is likely to happen, how much time is allocated and whether treatment is proposed that day. If you have pain or swelling, mention it immediately so the team can assess urgency.
Agree how you would like explanations to be given. You might prefer each step described in advance, or only a brief explanation followed by an opportunity to ask questions. Ask about a hand signal to request a pause and how the dentist will respond when it is safe to stop.
Communication, a sense of control and an individually tailored approach are central to the NHS England guide to dental anxiety management.

A message you can use before your visit
“I would like to arrange a dental assessment, but I feel anxious about treatment. My main concern is [your concern]. Could we discuss what will happen at the first appointment and agree a way for me to request a pause? I would also like to understand the treatment options before deciding.”
Add any current symptoms and your preferred language. If you would like someone to accompany you, ask the clinic whether this is possible and at which stages. Leave time for this exchange before your appointment rather than relying on a rushed conversation at reception.
Does anxiety mean you need sedation?
Not necessarily. Depending on the person and the severity of the anxiety, support may include communication techniques, behavioural approaches or referral for psychological help such as cognitive behavioural therapy. Conscious sedation may be considered in appropriate cases after assessment. It requires suitable clinical arrangements and does not replace informed consent. These options are outlined by NHS England.
Do not assume a particular sedation service is available simply because you are booking dental treatment. Ask what can actually be offered, who provides it and whether a separate assessment or referral is needed. If sedation is planned, obtain the preparation, escort and recovery instructions directly from the treating team before arranging your day.
Plan around your life in Istanbul
For residents, ask whether the treatment can be divided into manageable appointments where clinically appropriate. Consider a time when you will not be rushing from work or trying to return immediately to an important meeting.
For international patients, explain both your anxiety and your travel dates before confirming a treatment schedule. Ask how many visits are proposed, where decisions will be made and what happens if you need more time. Include space for discussion and review when considering how many days to stay in Istanbul.
Frequently asked questions
What if I have not visited a dentist for years?
Tell the clinic that you are returning after a long gap. Start with an assessment and ask for the findings to be explained in order of priority. You do not have to arrive knowing which treatments you need.
Should I agree to a full treatment plan before travelling?
Ask which parts are provisional and which require an examination. Make sure you understand the alternatives, expected costs and opportunity to ask questions before giving consent.
Can I ask for a pause during treatment?
Yes. Agree a signal beforehand and discuss how pauses will work during the proposed procedure. Knowing the arrangement in advance may help you feel more involved.
